Seems like you covered all of the bases so my only suggestion is to try
making it again and make sure it's under 30 seconds.

Another thing to try (try this first, actually) is to uncheck the syncing of
ringtones in iTunes, sync, check to sync ringtones again, and sync again.
This actually cured one of the ringtones I was having trouble with (which,
ironically, was one of the two I had actually purchased--merely as a test).

Oh yea, I like to increase the volume in the track before turning it into a
ringtone. My ringtones are *loud*. I find this works better if I do it
outside of iTunes. I use iPhoneRingtoneMaker as it was the only game in town
last summer. I only use it to make song snippets out of full tracks, not to
send to the iPhone (though the latest version tries to force that on you). I
often will add an effect too, such as a flanger effect, to my ringtone. Can
make it stand out. Once I'm done I just use the m4r rename trick to get them
into iTunes, ergo the iPhone.
