Hard Reset of vx-8560

I broke the screen so I can not see what I am doing.
I need the directions in numbers (press 2 press 4 etc...)
I got my new phone and I want to delete everything on my old phone - before I send it back to Asurion. How do I do this? Maybe there is a secret pin hole for reset??
