Broken Chocolate

I got my chocolate a few moths ago and it was fine, but sometimes when i tried to watch videos, the screen froze. Today I was charging my phone and i tried to watch a video. It froze, so i took out the battery like usual, only it was still on the charger. After i powered it back on, it said "FTM Mode" and it was reset to factory settings. I tried re-activating the phone, only now it says "Service Required" and i cant make any calls. I really need help.
