I said it depends, because the question is highly debated. Your reception with an internal antenna is affected more by the way you hold your phone because if your hand is covering the area of the intenna it will interfere with reception, but I've held one beside the other and they both have about the same reception as long as you hold the internal antenna the right way.

I put it depends and I wanna give an example.
I haven't had much compliants about the moto pebl or razr as far as reception goes. However, anyone ever deal with a Siemens CF62T???

i have been involved with radios for a long time ( a ham for about 10 years ) and an external antenna will always be a better choice and using it fully extended is best
with that being said it will still depend on the phone you are using . If it works fine for you being an internal antenna then no worries but if that phone is not offered with an external antenna then how could you compare them . So bottom line is if the antenna works for you great but if you are running into reception problems then you may need to find a phone with an external antenna to get the reception you needIf i helped you some +rep would be good

Razr v3c Reception
I have the Motorola Razr v3c from Verizon and with it's internal antenna I get reception that's just as good as I've ever had with any of my phones that have external antennas.

I have the MOTO V300 and have NO Problem with the antenna.. i saw someone at the gym today with the antenna you have to pull up and it almost looked like an old rotary phone.. i joked with him that he need to get with the times..hehehe....
