Try last 4 digits of SSN of the account holder.
If that doesn't work, call the carrier back and ask for the unlock code. The sales rep should have provided it to you when you bought the phone. Phones are shipped in a locked state to discourage thieves from intercepting them in shipment and running up a big bill in your name.
