Screen Locked SE W350a Phone

My son locked my phone and he forgot the code. I called AT&T (my service provider) and they gave me the PUK but I can't use it yet because when I turn the phone on, it asks for the phone lock code. How can I bypass this to get to a screen where I can reset the phone to the factory settings?
