I have an unlocked quad band phone (850/900/1800/1900) and am looking for a
global SIM with the following characteristics:
-UK phone number
-Free incoming calls to the UK, France and Spain
-Voicemail
-Credit that doesn't expire in 30-45 days
-Doesn't use 'callback' for long distance calls placed from the phone
-Inexpensive calls to the USA
-Top-up from the Internet
Can anyone in the group recommend a SIM with those characteristics?

Good luck. When you excluded the use of a callback system you pretty much
sunk the boat.

As Donald Newcomb already replied, it isn't going to be as simple as
you hope, and study PPGSM a bit.
>From the UK only, there are quite good rates for USA: an add-on
option on T-mobile at 4p/min, or Orange CallAbroad or MobileWorld at
5p/min. MW is an mvno from Carphone Warehouse, a large dealer chain.
For roaming, of actual UK networks, look at Vodafone and O2.
Vodafone has a scheme called Passport, which uses the home tariff plus
a connect fee of 75p per call; thus incoming calls cost just that, and
outgoing will be 30p or 10p off-peak. But that is to the UK, so you
will need another arrangement as well
O2 has 3 roaming deals, My Europe, single country - Spain and My
Europe Extra. The first has all calls (in and out) in about 30
countries at 35p/min. The others are similar, having incoming calls
free, outgoing to the other European countries 25p/min; My Europe
Extra costs a moonthly fee of £10 amonth, the Spanish version £5 a
month. Again you will need to make use of another arrangement to reach
USA cheaply.
You could set up a callthrough account to reach USA, maybe a calling
card with UK access, or maybe a Betamax VoIP account like Voipcheap or
Voipbuster. This would need to be set up with the UK as home country,
then you have a callthough access that will cost 1 eurocent a minute
on top of the UK-bound cost.
Or you could use callback to the roaming UK SIM, to get below the
indigenous SIM tariffs.
There are plenty of Isle of Man numbered global SIM brands, but all
global SIMs use callback. Many providers, but not all, count Isle of
Man mobile numbers as a UK number.
As you can see, you will probably have to resign yourself to using
callback and/or callthrough arrangements, but this will allow you much
cheaper calls.
If you are going to other countries besides those mentioned, then look
on the operators pages at PPGSM; for example Blauworld in Germany has
cheap rates for direct-dialled int'l calls, and there are several in
other countries
