In India (and elsewhere?), all CDMA providers hardwire the ESN right
into the phone. What I want to know is is why? Why cant that be put
into something like the sim? Is this just a marketing decision or is
there something technical that forces providers to do this? India uses
the CDMA2000 1x standard. Does the standard specify anything in this
regard?

I think it's more marketing to force the locals to buy from one
provider rather than off shore. CDMA does support the equivalent to a
SIM card. called R-UIM
