Purchased Sanyo 8200 blue on-line as the Sprint stores near me had none. I
was supposed to get the mail in rebate, and phone delivery was delayed
awaiting the out of stock rebate form. I called back the sales rep and said
deliver the phone and when the forms come in, mail it to me. So it was
"duly" noted on my account to send the phone first and rebate when it
    arrives.

True to form, Sprint billed me another 309.00 and change for another
purchased phone, which I received 2 days later. I called them and got the
dupe taken care of, and all was well. But I STILL haven't gotten my rebate
    form - it's been about 3 weeks.

I called today and they switched me to a rebate specialist, who told me to
write all the pertinent info on a piece of paper (name , address, sprint
phone #, phone/model brand, purchase price, and email address) with the
packing slip and mail it in. I asked if I should indicate I'm looking for
the rebate and she said the address I'm sending it to is set up for rebates
    only and they will know.

This sounds kind of ridiculous, but hey, if it gets me my $150.00 back, then
    I'll be happy.

Write to [email protected] apparently they now have more authority
to solve problems than they used to.

I have a mail-in rebate form here (LIT66882) that I picked up at an
Office Depot. Email me a mailing address and I'll mail it to you.

By the way, the form says it has to be mailed by 7/17/04.
