I reported a lost phone to Nextel; Nextel in turn, passed me on to an
insurance agent who refused my claim. When I challenged the refusal the
insurance agent said that this information was in Nextel's insurance book. I
said "I do not have it", agent "Too bad, its your responsibility to ask for
    the book".

I have had my phones over two years with insurance and never, ever, had I
heard of an insurance book. Is there a solution?

    Thanks for any reply

What information? I have never heard of an insurance book.

Yep, insurance plan depends on which plan you chose. Also the state you live
in also depicts the plans and charges for the insurance. There is onle one
loss/theft plan Nextel uses and I think thats Signal Insurance.

I've never been impressed with their insurance plan. Even if you had
been covered, you probably would have received a refurbished
replacement at best. They can send you a returned phone someone
    upgraded if they choose.

Your best insurance plan is to save the money yourself and when you
find the same phone on e-bay, buy yourself a spare.

not just the monthly fee but if you sign up for the lost or stolen plan
there is a $39 deductible. The way I looked at it, 3.50/month *12 months
(average time till I lose my phone) is about $42, that plus the deductible
is about $80. By then it won't be worth it to replace my phone with the
same model, I would rather just buy the newest, prettiest model.

Anyone disagree with my reasoning ?
