Do US cell/wireless carriers charge for incoming calls?
a) International incoming calls?
b) Interstate incoming calls?
c) Intrastate incoming calls?

Well, no.
US Cellular I believe offers free incoming calls NO MATTER where the person calling you is calling from. However, you must be in a local calling area (which is determined by their map) in order to get this feature. You must also have local service in your area to activate service with them. You must be on a qualifying calling plan to get this service.
Other carriers like Metro PCS offer free calls period as long as your in their local coverage area. If you're outside of it, you'll be billed roaming (and if they're calling you from another country, you'll be billed international rates no matter what). Also, most unlimited plans include unlimited domestic (U.S. only) calls sent/received.
