Every SIM chip has its unique PUK code. The provider should tell you if you
ask. When PIN is entered incorrectly 3 times, the PUK is required to reset
    the PIN.

PUK code is unique to each SIM. What security would there be to have
the same PUK code for every SIM. You in fact wouldn't have any SIM
security at all if that was the case.

from what i have been told and from memory... PUK code changes on a regular
basis from network provider and nokia.

It depends on what kind of phone you have and what network u were on.

i.e am on orange and use nk7200.. i would have to tell them its for a nokia
phone.. and they will give me the puk code. Nokia relases the general codes
then the code in question will be given to you. Each code is unique as.. it
can be the same code from one month for my nokia 7200 and change the second
month. but code for a nokia 7200 on tmobile will be different than one given
    to me by orange.

This will ensure that the phone is unlocked again and can be used with my
    SP.

No, the PUK code is a SIM security feature and doesn't affect the lock
status of the handset. In addition, the PUK for each SIM card is fixed and
    never changes.

In O2's case the Puk is generated by the phone number as is on their site.
So maybe someone needs to make a Puk Generator. So many people lock
themselves out, I just go on O2's website, but then again my PIN is
    unforgettable.

I don't think that's quite right, but instead that O2 hold an accessible
database you can access where they can tie the phone number to a particular
SIM and PUK code. Mind you, people have been trying to write a PUK
calculator for a long time and no-one has succeeded.
