I have a Motorola V557 cell phone. It's only about a month old and works well for the most part, but I noticed that occasionally, contacts are deleted from my address book. This seems to specifically happen when I add a voice name to some contacts; often times I then find that the previous record in the list is gone. Is there any way to fix this?

Are you saving the contacts to your SIM card or to the phone's memory?

Try copying the contacts (that you want voice tags on), to the phone's memory, then add the voice tag to it. Make sure it's saved and that you don't use BACK on it.

They were stored on the phone, as voice names can't be stored on the SIM card.

I also didn't use BACK or anything. I tested it again by editing the voice name of the records that caused a deletion last time, and it happened again each time, so there's some sort of pattern to the problem, but I can't figure out what it is. But it definitely seems to be a bug in the phone's software.

Always have to ask, as you can never know who knows what. Anyway... I'd give the carrier a call, just fiddled with mine and can't make it not save a voice tag, or delete it after rebooting the phone.

So yes does seem like a problem with the phone itself. Especially if you have plenty of room to save the voice tags in the first place. And since the phone is only a month old, there should be no problem with getting it replaced as an exchange.
