Their phone is off or they are using the phone and have turned off Call Waiting.
I enter the call waiting off code, for certain people I call, especially if I know I will been needing to write things down during the call, just so I don't get that doop doop call waiting tone.
Nothing is worse than having to ask someone to stop and wait or to repeat the info, if you get the call waiting beep.

The person you're calling could also be on a 3-way call or in an area that does not have reception. The most common would be either the phone is off or they are on the phone....or maybe they just don't like you and have their phone set to send you straight to VM by going under Security settings>restrict incoming calls. I just did that to myself and it worked like a champ...never liked talking to myself anyway!
