This worked on the LG Cosmos 2 and the LG Octane.. Thank You!!!!!

I Have A LG UX-355 Phone with a lock code on it and i don't know the phone number on it because the activation on the phone has been discontinued...its also a U.S. Cellular model....can anybody help me with info on how to get the code....any suggestions will be helpful.......thanks in advance!
