I just found this site and searched a little bit without finding anything that might help, so I apologize if this has already been answered.

This morning, while trying to create some sort of PIN that anyone finding my phone would need to know in order to use it, I locked my SIM card

It asked for a PUK code and told me to call my service provider (AT&T). The PUK code they had for my phone didn't work and they basically told me that I was out of luck. I got a replacement SIM card and my phone now works.

However, the big problem is that I lost all of my stored phone numbers. Is there any way that I can somehow retrieve though? ANY help is greatly appreciated...and let me know if you need any more info!
