They are refurbished, not new. They cost half the original new cost.
Subject to availability, on a store by store basis.

6-8 years ago I called *2, reporting a problem with the phone. A new
(didn't seem refurbished) unit appeared in the mail with a prepaid
return package. I dialed *2 to swap the account to the new unit,
returned the old one, and that was it. No more?

Its a replacement of whatever sprint feels like giving you. You'll get
a referbished phone and changer and nothing else. If your phone is no
longer made, sprint will probably give you a entry level cheapo phone
of the same brand. Example: spring replaced my samsung mp3 phone with
an a500. A $160 phone with a phone given free with a new contract.

It is for the Treo 650, carried the replacement insurance since it was such
an expensive phone. The insurance is the same on cheap phones or expensive
phones. I don't carry any insurance on my cheap phones. I have a 5 phone
    account.

I might add that they replaced the phone since the screen went bad with a
    brand new phone.

While I would definitely recommend against insurance for cheap phones on
ANY other carrier than Sprint, it appears that Sprint's attitudes
regarding warranty work have changed post Nextel merger. While older
customers appear to be grandfathered for now, Sprint is the only Tier 1
carrier that will tell you to take your phone to the manufacturer for
service if there is a warranty issue, and will not handle the
repair/replacement... UNLESS you are paying the $6/month for Total
Equipment Protection, in which case they then appear more than happy to
service/replace the phone for a defect-related cause, warranty or not.

From what I've heard, the phone manufacturers (Sanyo, Samsung, LG and
most definitely Palm) are unprepared for this, and will often try to
push you right back to Sprint to get your phone fixed, who will again
turn you away (unless, of course, you're paying $6/month for the
    insurance...).

I don't know about warranty service without the insurance, I do pay the 6 a
month on my pda phones. I had an older model treo 300 with the flip lid to
turn the phone on and/or protect the screen. One hinge broke and they
replaced the phone with an apparantly new treo 600.
