I purchased for my daughter... (Per past threads) a samsung Model e315. I was told I had 7 days to request the insurance.
I got my bill today and it made no mention of the insurance so I phoned. They said I never reqested it.
Checking my emails I showed I requested it on November 28th
    Received the phone November 22nd.

I sent them emails showing the correspondence. They said no this was insurance I had requested on my other number... Which I have had the phone for 2 years! I told them, per their own admission and the email contact that they only allow 7 days within the time frame of the phone.

    They wont budge!!

I have the correspondence! They say they just told me I had insurance on my other number.. this is a clip from my original request on the 28th.

This is what I wrote on the 28th
My text messaging including aim is unlimitted, correct? Also, please add

Hopfully this post reaches you in time. Remind them you have 14 days to cancel your new activation without having to pay the early termination fee. If that doesn't work get a manager on the phone.
