Anyone have a view what the best bluetooth eheadset is out there at the
moment? I need something loud and that preferably sits 'in ear' rather than
'over ear', although if it's loud enough to shut out motorway noise either
will do. Tried the Jabra BT250v but not impressed at all.

I don't know about "best" headset, but my "cheap" Nokia HS-36W works for
me. I've had it several months and so far it still seems fine. You can
see details of one at http://www.mphone.co.uk/acatalog/nokia-hs-36w.html
(no affiliation; simply a quick google).

It's an over the ear job that fits comfortably enough with the arm of
my glasses. Oh yes, fits over either ear.

Single button function: quick press => voice speed dial or hangup; longer
press => redial last number; really long press => switch off. Volume is
    on a separate rocker switch.

Battery life is reasonable. I think I charge it up about once a
    week, or so.
