MetroPCS is a regional 1900 CDMA carrier in just 5 cities (in GA, FL,
and CA), including the SF Bay Area. They offer UNLIMITED calling for
$35/mo. from within their SF metro service area to a slightly larger
area, or for $40 you get LD, so you can call from within their SF metro
service area to anywhere in the US. The phone does not operate outside
of their service area.

It's not available everywhere, or to everyone. Runs=20
something like $45-$60/month for unlimited home local,=20
local toll, and state-to-state long distance, then $139 for=20
unlimited PCS minutes *on one phone*. Each add-a-phone is=20
(recently went up, so I think it's) $100/month. Plus=20
Vision service for $15/month primary-$5/month secondary.
Expensive, but if you really *do* need that kind of minute=20
availability, maybe not outrageously so.
